Complete the consent form to confirm that you agree to take part in the project.

Who can take part?

  • Black women over 18+ years old

  • Women who have or have had gestational diabetes in pregnancy

  • English speakers

  • Residents of Hyson Green, Forest Fields, Bobber’s Mill, Radford, St Ann’s and The Park
